Bolton Clarke Residential Aged Care Opportunities – Mandurah WA
Salary: $15,900 a year plus $2,650 meal and entertainment allowance. Full‐time position based in the Mandurah office with travel to Mandurah and Bunbury.

Clinical Care Partner – Role Overview
As a Clinical Care Partner you will support older Australians to live independently at home by managing government‐funded Home Care Packages. Your role combines client visits, office‐based work, and leadership of holistic care in the community.
Responsibilities
- Provide comprehensive client assessment and develop a care plan to support the client at home
- Deliver advanced holistic care skills within a case‐management framework, including wound care and clinical assessments
- Demonstrate an understanding of the target population and their needs, from care coordination to complex case management
- Oversee budget management for Home Care Packages and monitor spend with clients
- Work within the legal and funding framework for Home Care Packages and be familiar with the Support at Home Program
- Take professional responsibility for the client's interface with the service system, including advocacy and supported decision‐making
- Participate in quality improvement processes, including case review and team meetings
- Coordinate with General Practitioners, hospitals, and allied health professionals to provide safe, person‐centred care
- Coordinate services across Mandurah and Bunbury – travel required across both regions

Qualifications
- Bachelor's degree in nursing
- Understanding of care at home packages highly regarded
- Current registration with AHPRA
- Australian work rights
- Police check valid within the last 3 years
- Current WA driver's licence and ability to travel across the region as required
- Current First Aid and CPR certificate
