Nepean Hospital, located at Kingswood is seeking a passionate and dedicated Registered Nurse to join our Heart Failure Team in a Temporary Part time role. This is a rewarding opportunity to provide specialised, patient-centred care to individuals managing chronic heart failure and contribute to improving their quality of life and long-term outcomes.
As a Heart Failure Registered Nurse, you will play a key role in the delivery of care coordination, patient education and chronic disease management for individuals living with heart failure. Working both independently and as part of a collaborative team, you will help guide patients through complex care pathways while promoting self-management and hospital avoidance strategies.
This role will see you provide comprehensive nursing assessments and evidence-based care for patients with heart failure in inpatient and outpatient settings will collaborate with Cardiologists, General Practitioners, Allied Health staff and other stakeholders to deliver integrated care.

What You Can Bring To The Role
* Current registration to practice as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Authority (AHPRA)
* Strong commitment to contribute to own and team professional development and act as a resource person and to mentor others within the speciality where required.
* Preferred experience in clinical teaching including preceptorship and / or student facilitation
* Demonstrated effective communication skills – written, verbal and eMR
* Demonstrated understanding of the issues that affect the health and well-being of clients with an acute/chronic cardiac condition and the impact of this on their families
* Able to perform effectively as a team member by liaising and consulting with other senior roles in the service as required.
* Demonstrated ability to exercise extended autonomy by holding decision making capabilities applicable in speciality clinic practice and service delivery.
