Seeking a highly motivated and reliable Care Coordinator to provide quality in-home care and assistance to our fast-growing client base.
Key Responsibilities
* Deliver services under various funding sources, including private pay clients, My Aged Care, and NDIS.
* Assess, plan, and review care plans as required, ensuring services meet clients' needs, goals, and preferences.
* Liaise with clients, family members, staff, and stakeholders to ensure seamless service delivery.
* Manage inside and outside sales enquiries, develop new referral sources, and maintain existing relationships.
The successful applicant will work collaboratively with our care team, providing exceptional support and guidance to clients and their families.
Requirements
* Strong connections with clients and their families.
* Ability to assign staff to clients based on qualifications, availability, and client preferences.
* Maintaining accurate records of incidents and feedback.
* Re-organising work priorities to meet the needs of a fast-paced environment.
Preferred Skills:
* Experience in assessing and planning care needs.
* Sound understanding of the Aged Care and Disability systems.
* Strong client focus, based on goals, preferences, and needs.
* Demonstrated experience managing staff.
* Current National Police Check, Working with Children Check, and Full Drivers Licence.
* Organisational, administrative, and time management skills.

* Conducting Client Care Assessments and Care Planning.
* Supervision and management of Care Staff.
* Discussing and promoting services with medical professionals and referral sources.
