Duties
* Provide support to clients based on their NDIS plan, implementing all supports within the client’s plan including formal, mainstream, and funded supports.
* Assist participants to negotiate provision of services and supports; monitor the delivery of services and supports and assist participants to understand how their NDIS Plan will fund supported needs.
* Provide time limited specialist support coordination to address high level risk and assist participants to connect with supports and build capacity and resilience.
* Providing crisis support to Participants.
* Use a strength-based approach when working with clients to establish a Support Plan with the client and other relevant stakeholders.
* Build effective relationships with clients and their families while maintaining appropriate and professional boundaries.
* Manage client issues and complaints with respect and fairness whilst following policies and procedures.
* Ensure regular contact with participants is maintained and recorded.
* Prioritize, manage, and administer a caseload / task in a coordinated, efficient, and timely manner.
* Ensure that information on relevant community resources is available to clients and appropriate referrals are made and followed up.
* Develop and maintain effective working relationships with stakeholders, other Disability services and Mental Health services, other non-government agencies and the National Disability Insurance Agency (NDIA)
* Complying with all relevant legislation and Codes of conduct
* Proven experience working with people with autism, disability and / or mental illness.
* Maintaining an UpToDate knowledge of the NDIS system and practices, be aware of and ensure all NDIS policy, procedures and guidelines are adhered to.
* Ensure appropriate NDIS Billing for Support Coordination is completed accurately and on time.
* Display a commitment to professional development, participate in and support the team in staff supervision, development and learning opportunities.
* Provide feedback to other service providers.
* Ensure client file documentation supports the standards for documentation and meets NDIS guidelines
