Summerland Support Services is currently seeking experienced and well organised Support Coordinators to join our team on a long-term basis, delivering exceptional support tailored to the diverse needs of our participants.
We have both full-time and part-time positions available.
Responsibilities include:
* Providing support coordination to NDIS participants with varying support requirements.
* Employing a person centred approach to enhance the capacity, choice, and control of participants.
* Collaborating with clients to develop and implement support plans, while monitoring progress and outcomes.
* Identifying and addressing complex challenges, barriers, and risks participants encounter, in conjunction with stakeholders.
* Demonstrating a solution focused mindset, encouraging innovative thinking.
* Contributing to organisational development and community networking efforts.
* Adapting to evolving participant circumstances.
* Cultivating a welcoming, inclusive, and respectful support network for individuals from diverse backgrounds.
Qualifications and experience required:
* Experience in NDIS support coordination or a similar role.
* Qualifications in community services, disability, or related health disciplines.
* Thorough knowledge of NDIS operational guidelines.
* Possession of a current driver's license and a comprehensively insured, safe vehicle.
* NDIS worker screen clearance and working with children check.
* Certificate of completion for the NDIS worker modules.
* First Aid and CPR certifications.
* Excellent interpersonal skills to establish productive relationships with various stakeholders.
* Proficiency in computer literacy and the Microsoft suite.
* Ability to work independently and proactively.
