About the role:
We are seeking a dedicated and compassionate Residential Care Officer to join our team at Gratifying Care Pty Ltd. In this role, you will be responsible for assessing the needs of our clients, creating and implementing personalised care and support plans, and providing hands-on assistance in both home and community environments. You will work closely with clients, their families, and external agencies to ensure holistic, high-quality care that aligns with our person-centred approach. This position is ideal for someone who is passionate about making a difference, skilled in building trust and rapport, and committed to empowering individuals to live with dignity and independence.
Key Responsibilities:
- Assess client needs through thorough interviews and evaluations, developing personalised care and support plans that reflect individual circumstances.
- Implement and coordinate tailored programs covering personal care, domestic support, and community engagement to promote independence and well-being.
- Monitor and document client progress, adjusting care strategies as needs evolve.
- Facilitate access to external services such as healthcare providers, NDIS supports, welfare agencies, and community resources.
- Identify broader community needs and resources that may benefit clients, including housing, training, and employment opportunities.
- Liaise with community groups, government agencies, and service providers to strengthen partnerships and raise awareness of available support.
- Deliver compassionate in-home care to elderly individuals and people with disabilities, in line with Gratifying Care’s person-centred approach.
- Support and educate families to enhance their capacity to care for loved ones in residential or community settings.
- Assist clients in addressing social, emotional, and financial challenges, fostering resilience and improved quality of life.
- Prepare reports, funding submissions, and service documentation for internal use and external stakeholders.
Requirements: .
- Certificate IV in Disability, Community Services, Aged Care, or related field (or higher).
- A minimum of 3–4 years relevant experience in residential care, community support, or disability services.
- Additional qualifications such as a Diploma of Community Services will be highly regarded.
- Proven ability to develop and implement personalised care plans.
- Strong communication and interpersonal skills, with the ability to build rapport with clients, families, and service providers.
- Sound knowledge of community resources, support services, and relevant legislation.
- Current First Aid and CPR certification.
- Valid driver’s licence and access to a reliable vehicle.
- Ability to work both independently and as part of a multidisciplinary team.
