**About the role**:
Reporting to the Virtual Rehabilitation Ward Manager, in your role as a Social Worker you will participate as a member of the Virtual Rehabilitation Ward team, which provides care to older people with complex medical, functional and/or psychosocial needs in their own home in lieu of an extended or avoidable hospital admission.
Duties for the role will include:
- Contribute to safe and timely discharge and improved independence and occupational outcomes for clients,
- Documenting assessment findings and interventions in a timely fashion,
- Liaising with the Senior Social Work and the Director, Social Work RAP about the provision of services and resources in the occupant's work area,
- Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers,
- Undertaking training as required to attain and maintain required competency of skills and knowledge applicable to the role.
**About you**:
- A degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers,
- Ability to undertake assessment and intervention with clients with a range of illness and disabilities relevant to older people and rehabilitation,
- Organisational skills including the ability to prioritise work, meet deadlines, delegate appropriately and demonstrate initiative,
- Experience in the provision of clinical Social Work services to clients with a range of illness and disabilities relevant to older people and rehabilitation,
