About The Role
The Medical Assessment & Planning Unit is a 23-bed unit with telemetry capabilities. The Model of Care includes a dedicated physician-led medical team, working with a multidisciplinary team to provide quality patient care through rapid assessment of admitted patients. The unit targets specific medical patients for early senior clinician assessment and collaborative multidisciplinary input, enabling safe discharge within an expected timeframe. We are committed to delivering excellent, patient- and family-centered care, focusing on continuity of care and health optimization.
Role Responsibilities
Deliver advanced clinical care in acute medical nursing, supporting and guiding team members.
Communicate effectively with healthcare team members, patients, families, and visitors to foster a positive workplace culture.
Build and maintain strong multidisciplinary relationships, contributing to team learning and development.
Participate in identifying improvements in work practices and policies.
Prioritize clinical and administrative duties, efficiently managing complex caseloads with appropriate reporting systems.
Provide clinical education and support at an advanced level, demonstrating team commitment.
Mandatory Requirements
Vaccination against, and ongoing immunity to, specified vaccine-preventable diseases as per Health Employment Directive NO. 01/16.
