About Heart to Hold Foundation
Heart to Hold Foundation is a values-driven NDIS provider delivering individualised, trauma-informed, and culturally respectful support to people living with disability and psychosocial challenges. We are deeply committed to working alongside Aboriginal and Torres Strait Islander communities, and we’re currently expanding our services across Darwin and surrounding areas.
We are looking for passionate, reliable, and culturally informed Support Workers who can provide respectful, empowering support to participants in their homes and communities.
About the Role
You’ll provide hands on, culturally safe support to NDIS participants, helping them achieve their goals in everyday life. Your work will make a real difference in the lives of individuals, families, and communities.
Key Responsibilities:
- Provide one-on-one support to individuals with complex behaviours in accordance with their individual care plans.
- Assist with personal care, mobility, and daily activities, including meal preparation, medication administration, and household tasks.
- Develop and implement behavior management strategies to support individuals with challenging behaviors in a compassionate, calm, and effective manner.
- Monitor and document the progress of individuals, noting any significant changes in their behaviour or needs.
- Promote social inclusion and engagement in community activities, fostering a positive and empowering setting.
- Work closely with families, healthcare providers, and other professionals to ensure the holistic care of individuals.
- Adhere to all health and safety regulations, including reporting any incidents, accidents, or concerns.
To Be Successful, You Will Need:
- Experience in disability, community, youth, or mental health support
- A kind and respectful approach to care
- NDIS Worker Screening Check (or willingness to obtain)
- Working with Children Clearance (Ochre Card)
- First Aid & CPR certification
- Driver’s licence and reliable vehicle
- Strong communication skills and cultural awareness
