Targeted Care Packages Support Worker
About the Role:
Targeted care packages are designed to provide flexible support solutions tailored to an individual's specific needs. By allocating funding to a particular child or young person based on their assessment, we can help prevent them from entering residential care and enable their transition into alternative placements.
This model offers personalized supports that cater to the unique needs of children and their carers, promoting family reunification, kinship, home-based care, lead tenant, or independent living arrangements.
Key Responsibilities:
* Supporting the keyworker team by collaborating effectively with all stakeholders involved in a young person's case plan.
* Providing emotional and practical assistance to meet the young person's needs, including responding to emergencies and supporting their personal development.
* Developing the young person's skills and abilities through education, training, and mentoring.
* Managing administrative tasks efficiently, ensuring accurate records and timely reporting.
* Offering practical assistance related to the young person's daily life, such as household chores and errands.
Working Therapeutically with Children and Young People:
* Directly engaging with the child or young person, their family, carers, and other relevant parties to facilitate positive changes that support the achievement of the child's or young person's case plan.
Key Selection Criteria:
* Relevant tertiary qualifications in social work, child and youth services, or a related field.
* A current valid driver's license and ability to drive company vehicles safely and efficiently.
* Knowledge of relevant legislation and policies, such as the Children, Youth and Families Act.
* Understanding of trauma and its impact on individuals, particularly children and young people.
* Experience working in the community sector, preferably with vulnerable populations.
