Community Nursing Opportunity
We are seeking a skilled and compassionate Registered Nurse to join our Community Nursing team. As a valued member of our team, you will provide high-quality nursing care to patients in their homes, promoting independence and improving health outcomes.
About the Role
This is a permanent part-time position working up to 16 hours per fortnight. You will be based at Deloraine District Hospital and work closely with our multidisciplinary team to deliver patient-centered care.
Key Responsibilities
* To provide safe, quality, clinically appropriate nursing care to patients in their homes.
* To assess patient needs and develop individualized care plans.
* To work collaboratively with other healthcare professionals to ensure seamless care transitions.
* To maintain accurate records and communicate effectively with patients, families, and colleagues.
Required Skills and Qualifications
To be successful in this role, you will require:
* A current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
* A valid driver's license.
* Excellent communication and interpersonal skills.
