As a Clinical Coordinator with Salvos Home Care, you’ll play a key role in delivering high-quality, person-centred care under the evolving Support at Home program - helping older Australians remain independent, safe, and well in their own homes.

About The Role
In this dynamic, community-based role, you’ll lead the clinical coordination and management of services for a portfolio of Home Care clients. Working closely with clients, families, and internal teams, you’ll ensure care delivery aligns with Support at Home principles, focusing on independence, reablement, and tailored clinical support.
This role involves regular travel across the Central Coast, with visits scheduled based on client needs.
You Will
Conduct clinical assessments, onboarding, and ongoing reviews for Home Care clientsDeveloping, reviewing and monitoring personalised care plans and quarterly budgetsProvide clinical oversight and respond proactively to changing care needsDeliver hands-on care where required, including wound care, medication management, PAS assessments and health monitoringSupport clients with more complex or higher-level clinical needs in collaboration with the broader teamIdentify risks and implement timely clinical interventions and referralsMentor and support Home Care Workers, promoting safe, high-quality care delivery
About You
Registered Nurse with current AHPRA registrationClinical experience in community or aged careStrong leadership skills with the ability to mentor and support care teamsUnderstanding of consumer-directed care and Support at Home principlesExcellent communication and relationship-building skillsStrong clinical assessment and organisational abilitiesCurrent driver’s licence and willingness to travelAlignment with and willingness to support the Christian values and ethos of The Salvation Army
