Social Worker Level 1/2 - Shoalhaven Transitional Aged Care Service
Be pivotal to the coordination of patient care to ensure optimal health outcomes for elderly clients in the community in collaboration with a multidisciplinary team.
Salary : $38.51 - $54.94 per hour + 12% super & salary packaging | Permanent Part Time 16 hours per week | Shoalhaven Memorial Hospital (SMH) | Applications close: 11 December 2025
Provide high quality social work service to patients, clients, consumers and their families/carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.
Key Responsibilities
* Provide the above social work service to the Illawarra Transitional Aged Care Service (ITACS).
* Maintain social work and case management responsibility for a number of cases until discharge.
* Early psycho-social assessments, care planning, education and advocacy for the needs of clients, families and/or carers.
* Liaise with community agencies, the client's General Practitioner, counselling and carer support.
* Work as part of an inter-disciplinary team providing low intensity therapeutic home‑based programs to frail elderly clients ready for discharge from sub‑acute hospital care.
Qualifications & Requirements
* Applicable vaccination requirements for a Category A role.
* Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
* Valid NSW Driver's Licence.
