Working in partnership with Karlup Aboriginal Corporation and Indigo Junction, the Booraka Danjoo Outreach Coordinator will lead a dedicated team delivering culturally safe, trauma‐informed, and person‐centred outreach services to people experiencing homelessness and vulnerable community members across Perth's North East Metropolitan Corridor.
The coordinator will provide strategic leadership, operational oversight, stakeholder engagement, and coordination of assertive and responsive outreach services. This role focuses on team leadership, collaboration with partners, timely information sharing, report writing, and high‐level service coordination.
The coordinator will ensure the delivery of high‐quality service delivery that empowers individuals and families to access housing, health, wellbeing, and community support services. This role plays a key part in strengthening collaborative responses to homelessness while fostering positive outcomes for Aboriginal and Torres Strait Islander peoples and the broader community.
About the Role
Working within a Trauma‐Informed Care framework, you will:
* Lead, mentor, and support a team of outreach workers.
* Coordinate assertive outreach and responsive outreach activities, ensuring timely and effective deployment of the team to meet service demand.
* Build and maintain strong collaborative relationships with community organisations, government agencies, and key stakeholders.
* Attend and actively contribute to stakeholder meetings, inter‐agency forums, and partnership networks.
* Coordinate timely information sharing and case conferencing with relevant services and partners.
* Oversee triage processes, escalation pathways and short‐term casework to individuals and families accessing the service.
* Support the team to deliver informal counselling, advocacy, referrals, and practical support to clients experiencing crisis and homelessness.
* Coordinate team responses to support people sleeping rough to access immediate crisis support, facilities, and essential services.
* Ensure the team effectively supports clients to access housing, health, alcohol and other drug services, community resources, and government supports.
* Build and maintain strong relationships with community organisations, government agencies, and key stakeholders.
* Monitor program outcomes, reporting requirements, service quality, and funding compliance.
* Ensure accurate client records, data management, and reporting requirements are maintained.
* Promote a culture of empowerment, self‐determination, and culturally safe practice.
About You
* A tertiary qualification in Human Services, Social Work, Community Services, Psychology, or a related field, with a minimum of three years' relevant experience, or demonstrated equivalent experience.
* A sound understanding of the principles of Trauma‐Informed Care.
* Demonstrated experience and competence working with people in crisis and experiencing complex needs.
* Experience leading community services, homelessness, outreach, or case management teams.
* Experience working within a case management framework, with a strong understanding of professional boundaries.
* Demonstrated experience providing advocacy, referrals, and support coordination.
* Knowledge of community, housing, and government support services.
* An understanding of the cultural and social factors contributing to homelessness and crisis.
* Experience supporting people affected by homelessness, alcohol and other drug issues, mental health challenges, and other complex needs.
* A strong commitment to empowerment, self‐determination, and person‐centred practice.
* Cultural competence and experience working with people from diverse backgrounds, including Aboriginal and Torres Strait Islander communities.
* High‐level interpersonal, communication, negotiation, and advocacy skills.
* Strong written communication skills, including report writing and record keeping.
* Excellent organisational, time management, self‐management skills, and managing a budget.
* Sound computer skills, including experience using databases, spreadsheets, and Microsoft Office applications.
* Current C Class Driver's Licence.
* Current National Police Clearance.
* Current Working with Children Check.
* Current First Aid Certificate (or willingness to obtain).
* Ability to travel throughout the Perth Metropolitan North East Corridor as required.
