Care Manager Position
* We are seeking a skilled and compassionate Care Manager to lead our team in providing exceptional care to our residents.
Responsibilities:

* Clinical Governance: Review clinical incidents, complaints, and changes to ensure compliance with legislative requirements and organisational policies.
* Person-Centred Care Planning: Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers.
* Medication Management: Oversee medication management in accordance with resident needs and legislative requirements.
* Liaison and Collaboration: Liaise with allied health, medical practitioners, and other services to provide optimal resident care.
Requirements:
* A degree in a relevant field (e.g. nursing, healthcare, social work).
* Experience in a leadership or management role within a healthcare setting.
* Excellent communication and interpersonal skills.
