We engage with individuals and their families to provide the best possible journey ensuring physical, emotional cultural and spiritual preferences are supported. The Role This role requires respect, empathy and cultural understanding in supporting Aboriginal and Torres Strait Islanders and their families on their end-of-life journey.In this role you will:Support Aboriginal and Torres Strait Islander people and families to access culturally safe palliative care servicesBuild trust and engage respectfully through in-person and virtual supportConnect clients with appropriate services and assist with culturally appropriate care planningAdvocate for clients and help them understand and navigate available supportsIdentify barriers to care and work collaboratively with health professionals to improve accessRaise awareness and normalise end-of-life conversations in community settingsStrengthen partnerships between Aboriginal health services and mainstream providersAssist with data collection, reporting, and program evaluationContribute to capacity building and cultural safety across services About You We're looking for someone who is passionate about supporting Aboriginal and Torres Strait Islander communities, has strong communication and advocacy skills and a deep understanding of the needs of Aboriginal and Torres Strait Islander people.You bring a strong commitment to confidentiality and culturally safe practice, along with an understanding of working respectfully with Aboriginal and Torres Strait Islander communities. Experience in healthcare or community services is highly regarded. What's on offer?
