Our organisation is seeking a highly skilled and experienced professional to support offenders in their transition from custody to community as part of our Initial Transitional Support (ITS) Service.
Job Description
* Provide high quality evidence-informed support for clients, ensuring they receive the best possible care during their transition period.
* Link clients with post-release support services, delivering innovative intervention strategies that support reintegration back to the community.
* Deliver targeted interventions that align with our case plan, providing outreach support for sustainable community reintegration and transitional support.
* Improve the immediate transition period following release by assisting clients to address issues directly related to offending behaviour.
* Operate within an evidence-informed strengths-based case management framework, utilising principles of Risk Needs Responsivity.
* Provide culturally safe, sensitive and appropriate service for all program clients.
