About the Roles
We are seeking dedicated case managers to join our SEWB team. Two positions are available:
* Aboriginal Community Case Worker
* Senior Case Manager
Appointment level will be determined based on your experience, qualifications, and demonstrated capability.

Aboriginal Community Case Worker
* Provide coordinated case management and SEWB education to female Aboriginal clients
* Work in partnership with GPs, clinicians and allied health for a multidisciplinary approach
* Focus on drug and alcohol assessment, treatment planning, individual counselling and support, and group programs for substance abuse recovery
* Deliver culturally appropriate SEWB education and early intervention activities
* Liaise with community groups, health professionals, and agencies
* Maintain information systems such as computer systems, Communicare Patient Information Recall System, data entry, statistics and other records required for the efficient and effective management of clients
* Participate in quality assurance programs to ensure safe and effective care provision
* Work collaboratively within a multidisciplinary team
* Perform other relevant duties as directed.
Senior Case Manager
* Lead and deliver coordinated case management, supporting female Aboriginal clients
* Drive program development and evaluation within SEWB
* Provide high level mentoring/leadership within the multidisciplinary team.
* Focus on drug and alcohol assessment, treatment planning, individual counselling and support, and group programs for substance abuse recovery
* Deliver culturally appropriate SEWB education and early intervention activities
* Liaise with community groups, health professionals, and agencies
* Maintain records in Communicare and actively participate in continuous improvement and quality assurance
* Work collaboratively within a multidisciplinary team
Essential (both roles)
* Demonstrated ability to work both independently (with minimal supervision) and in a multidisciplinary team
* Use initiative and meet deadlines.
* Strong communication with Aboriginal and non‑Aboriginal people
* Ability to observe strict confidentiality regarding all information pertaining to all client/s of the Organisation and organisational matters
* Willingness to undertake further education, training and other professional development activities relevant to the position.
* Effective collaboration across disciplines.
* Demonstrated ability to effectively, accurately and appropriately utilise information technology, in particular Microsoft Office products and data entry programs
* Experience in effective case management
* Experience in working with Aboriginal people
* Experience in working in regional communities
* Knowledge of Social and Emotional Wellbeing and Mental Health principles and initiatives, Primary Health Care, Cultural Safety, Social Justice, and relevant Equal Opportunity & WHS legislation; knowledge of local services, community needs and kinship structures.
Role‑specific
* Case Worker: Demonstrated experience working with female Aboriginal clients with AOD dependency and facilitating recovery‑focussed group programs.
* Senior Case Manager: Substantial experience in case management with female Aboriginal clients and capability to contribute at program level/mentor peers. Experience providing person‑centred supports that enhance strengths, resilience and quality of life.
Requirements:
Aboriginal Community Case Worker
* Have or be willing to obtain competencies and qualifications as an Aboriginal Health Worker by completing a nationally accredited Certificate III or IV in Aboriginal and/or Torres Strait Islander Primary Health Care, or other relevant qualification
Senior Case Manager
* A Certificate IV or above in a relevant health-related area – Aboriginal and/or Torres Strait Islander Primary Health Care/Mental Health/Community Services

* Aboriginality is a genuine special measure requirement of this position with the exemptions claimed under Section 65s of the SA Equal Opportunity Act 1983.
* Competencies necessary to practise in the particular practice setting
* Must hold a valid, unrestricted driver's licence
* A current DCSI Working with Children Check (WWCC) issued within the last 5 years
* A National Police Certificate issued within the last three years.
* Evidence of compliance with PLAHS staff immunisation requirements including an annual influenza vaccination.
