As a Registered Nurse with Right at Home, you will have the opportunity to make a real difference in the lives of our clients. We are seeking a qualified RN to join our team and provide compassionate, respectful and high-quality holistic nursing care in Aged, Disability, Transitional & Palliative Care and/or Hospital in the Home.


About the Role

This is a Part Time (various hours) position working in the Perth Northern and Perth West Coast suburbs. Your primary focus will be on delivering clinical services, supervising non-clinical team members and conducting client care assessments. You will also be responsible for intake processing, clinical care planning and reporting, as well as coordinating the provision of quality person centred care according to the client's needs and service plans.


Key Responsibilities

* Clinical delivery and supervision of non-clinical team members in a community setting
* Intake processing, clinical care planning and reporting
* Conducting client care assessments with a focus on Consumer Directed Care
* Coordinate the provision of quality person centred care according to the client's needs, service / care plans and available funding
* Clinical in-home services include care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, health and diabetes management
* General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation


Requirements

To be successful in this role, you will need:

* ESENTIAL
o Qualified Registered Nurse
o Current AHPRA registered
o First Aid & CPR
o Australian National Police check
o Professional Indemnity Insurance
o Experienced in assessing and planning care needs within the community
o Valid Driver's Licenceand reliable presentable car
o A smart mobile phone (iphone / android )
o COVID Vaccinations
* PREFERRED SKILLS
o A passion for high quality client-focused care in a community setting
o Cert IV in Training and Assessment - highly desirable
o Exceptional communication skills
o Able to work autonomously or within a team environment
o Excellent time management and organisation skills
o Commitment to ongoing professional development
