The Senior Chronic Care Worker - Illawarra will support the Manager of Aboriginal Chronic Care (ACCU) to implement the Models of Care for Aboriginal people in Wollongong and Shellharbour hospitals and Aboriginal people at risk of developing a chronic disease living within the Illawarra Region. The position will support the implementation, monitoring, and evaluation of Aboriginal Chronic Care initiatives involving health promotion and education to meet the needs of the Local Aboriginal community. While fostering a multi-disciplinary community health team approach. The role will be required to work in a collaborative and integrated approach with hospitals and other chronic disease services to ensure ACCU programs meet clinical and national accreditation standards.

This position will support Aboriginal people who access programs and services within the Illawarra hub including Wollongong Hospital, Shellharbour Hospital, and Illawarra outreach locations.

- Applicable vaccination requirements for a Category A role.

- Current driver's license Class C and ability and willingness to travel within the LHD for work purposes.

- Corroboration of Aboriginality and community

- Must be of Aboriginal and/or Torres Strait Islander descent (NB applicant's race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW) and have demonstrated knowledge of current Commonwealth, State and Local Aboriginal health priorities and ability to effectively and sensitively liaise and effectively communicate with local Aboriginal and Torres Strait Islander people and communities and agencies.

This is a temporary leave relief position until 9 May 2027 with the possibility of extension.
