Palliative Care RN - Home-Based Support
We are seeking a compassionate and skilled Registered Nurse to deliver complex nursing care and patient support for palliative clients in a home-based setting.
Key Responsibilities:
* To provide high-quality, person-centered care to patients with palliative needs
* To work collaboratively with multidisciplinary teams to develop and implement individualized care plans
* To conduct thorough assessments and evaluations of patient needs, identifying areas for intervention and improvement
* To communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions
Requirements:
* Current AHPRA registration as a Registered Nurse
* At least 2 years of experience in Palliative Care or a related field
* Strong knowledge of symptom management principles and practices
* Excellent communication and interpersonal skills
