Occupational Therapist Level 3: $ $60.86 per hour

Speech Pathologist Level 3: $ $60.86 per hour

In addition to the base salary all role attract 12% Superannuation + Salary Packaging

An exciting opportunity exists to join the established Disability Resource Team within South Western Sydney Local Health District as a Disability Resource Coach (Disability Navigator). In this role, you will make a meaningful difference by improving the care experiences of people with disability, their families and informal supports across SWSLHD facilities and services. You will work collaboratively with multidisciplinary teams and external stakeholders to identify and address disability‑related needs, establish reasonable adjustments and support safe, coordinated discharge planning. Through proactive care navigation and hospital avoidance strategies, you will help strengthen continuity of care between admitted and community settings, reducing the need for acute health responses and supporting better health outcomes for people with disability.

You are passionate about improving health equity and advocating for people with disability within complex healthcare environments. You bring the ability to work collaboratively with clinical teams and external partners, providing guidance, leadership and practical advice to support the diverse needs of people with disability presenting to hospital settings. You are confident navigating systems of care, contributing to coordinated transfer of care, and building strong networks across SWSLHD and community services. You are organised and accountable, able to meet the duties and reporting requirements of the role while contributing to service improvement initiatives that enhance care experiences for people with disability and their carers. This position is offered at 0.4 FTE, up to 16 hours per week, and is primarily based at Bankstown Hospital.

Selection Criteria

To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

1. Relevant qualification/ registration in one of the disciplines outlined in the position description.
2. Demonstrate your in-depth understanding of the health and disability sectors as a Health Clinician supporting clients within the National Disability Insurance Scheme (NDIS). Please provide examples of how you have coordinated care and navigated systems for individuals with complex needs—particularly those exhibiting behaviours of concern—across multiple services and agencies.
3. Describe your understanding of the principles of access and reasonable adjustment for people with physical, cognitive, communication, psychosocial, and sensory disabilities within healthcare settings. Please provide specific examples of how you have identified barriers and implemented practical adjustments to ensure equitable participation and inclusion of patients or consumers throughout their healthcare journey.
4. Do you have a current Australian C-Class driver's license (including P1 or P? if you hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
