Care Support Officer Job Description
At our facility, we are seeking a compassionate and detail-oriented Care Support Officer to join our care team. This full-time opportunity is available in Shepparton, Victoria for an individual who shares our mission of providing a safe, supportive, and welcoming environment for our residents.
Job Responsibilities:
* Conduct AN-ACC assessments in collaboration with care staff and leadership to ensure high-quality outcomes.
* Design and implement best-practice care plans that reflect individual needs and values.
* Undertake Resident of the Day (ROD) reviews alongside the Facility Manager to drive evidence-based decisions.
* Ensure timely completion of assessments and updates as required.
* Consult with residents, families, Div 1 nurses, and external providers to guide care planning and deliver resident-centered care.
* Lead continuous improvement initiatives through audits and feedback loops to enhance care delivery.
* Support carers and provide emotional, social, and clinical support to residents, managing specialized care areas including mobility, nutrition, wound care, pressure care, continence, etc.
* Maintain thorough documentation and communication across care teams and families.
* Complete HELF Checks and coordinate required training with PAL and P&C for new staff.
Essential Skills and Attributes:
* Registered Division 1 Nurse with current National Practicing Registration.
* Excellent problem-solving and care planning capabilities.
* Strong working knowledge of the needs of the elderly person residing within a residential care facility.
* Exceptional problem solving, assessment, and care planning skills.
* Excellent AN-ACC operational knowledge.
* Self-motivated, proficient with computerized documentation systems and records.
* Strong interpersonal skills with the ability to build and maintain working relationships within a multi-disciplinary team.
