About the Role
This pivotal position requires a collaborative and supportive approach to engage and empower young people as they navigate their journey towards independent living.
Key Responsibilities
* Work closely with a multidisciplinary Care Team to address the complex needs of young people in out-of-home care and adults who have exited the system.
* Utilise evidence-based Therapeutic Crisis Intervention techniques to facilitate de-escalation and help young people develop coping strategies.
* Develop comprehensive plans to support young people's transition to independent living, ensuring engagement with community resources and support services.
* Foster opportunities for peer groups and networking to promote social connections and relationships.
* Maintain strong relationships with significant family members, friends, community organisations, and government agencies to drive lasting connections and positive outcomes.
* Support and promote engagement with local Aboriginal and Torres Strait Islander Communities and Community Groups.
Requirements
* Relevant tertiary qualifications (3-year bachelor's degree) within the community service sector; Social Science, Social Work, Psychology, Human Services, and/or relevant experience.
* A current QLD Driver's Licence.
* Demonstrated experience working with young people, supporting them to articulate their goals, strengths, and aspirations, and assisting in achieving these goals.
* Demonstrated experience engaging mainstream services and reconnecting with Aboriginal and Torres Strait Islander clients and their families.
* Demonstrated experience in person-centred case management, coaching/mentoring service delivery, and managing complex caseloads.
* Demonstrated understanding of factors impacting young people's transition from out-of-home care to independent adult life.
