Working with external care teams and families, the role is responsible for providing high-quality, responsive, and person-centered support to assist participants enhance their everyday living skills, gain access and participation in community to achieve their goals, enhance their quality of life and physical and mental health.Supporting clients with personal care, community access, and daily routinesWorking in participants' homes or in our purpose-built Vertical Villages (SIL/SDA)Managing complex needs including behaviours of concern, trauma, forensic backgrounds, and high clinical needsAdministering medication and following detailed health support plansImplementing behaviour support plans and using de-escalation techniquesProviding meaningful, person-centred care to NDIS participantsCollaborating with clinicians, case workers, and familiesAbout YouYou will thrive and succeed in our team by bringing the following experience to this position:Minimum 6 months' experience in disability, aged care, mental health, or related fieldsCertificate III/IV/Diploma in Disability, Individual Support, Aged Care, or similar (preferred)Bachelor's in Nursing is highly desirableDemonstrated experience with high physical and clinical needs, including medication administrationStrong understanding of professional boundaries, restrictive practices, and behaviour supportEffective written and verbal communication, including transparent case note writingAble to set priorities, meet deadlines, and work under pressure with minimal supervisionStrong interpersonal skills to work both autonomously and as part of a teamNDIS Worker Screening, First Aid, Driver's Licence, and insured vehicleApply now to build a meaningful career supporting independence, inclusion, and choice.Seniority levelEntry levelEmployment typePart-timeJob functionOtherIndustriesHospitals and Health Care
