About Us

Nepean Blue Mountains Local Health District (NBMLHD) is a wonderful place to expand your career and grow your skills and knowledge. As a recognised leader in the healthcare industry, we provide a range of public health services to the Nepean, Blue Mountains, and Lithgow Region.

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).

We are seeking a passionate Community Health Care Registered Nurse to deliver high quality, person centred care to individuals, carers, and families. In this role, you will provide nursing assessment, planning, implementation, and evaluation using evidence based best practice and the principles of Primary Health Care. Working collaboratively with clients, families, GPs, and multidisciplinary teams, you will ensure care is flexible, responsive, and aligned with the Australian Aged Care Quality Standards.

You will provide services such as wound management, palliative care, chronic disease management, and continence care across community settings, including homes and clinics. This position offers variety and operates on a 7 day rotating roster. Responsibilities include supervising Enrolled Nurses, coordinating care for complex and high risk clients, advocating for diverse cultural needs, and mentoring staff and students. Your commitment to ethical practice, adherence to NBMLHD policies, and effective resource management will ensure optimal outcomes for patients and carers.

What You Will Bring To The Role

* Current registration as Registered Nurse Division 1 with AHPRA.
* Demonstrated recent experience and clinical skill in general nursing as a registered nurse (including completion of a new graduate RN program or 12 months full time equivalent experience working as a Registered Nurse).
* Demonstrated ability to organise and prioritise work and an ability to effectively work both within a team and autonomously.
* Demonstrated evidence of being able to provide person centred care and an ability to liaise with other health professionals to facilitate case management and care planning.
* Demonstrated effective communication skills (written and verbal) and interpersonal skills.
* Demonstrated commitment to professional development, evidence based practice and an awareness of changes within the health care environment.
* Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications (e.g. Cerner) and commitment to undertake training in electronic health record systems as required.
* Current NSW State driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD.
