You will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GP's. You will conduct polypharmacy medication reviews and SMRs, ensure better access to healthcare, and support practice staff with prescription and medication queries. You will also reconcile and review medications post-discharge, conduct medical related audits, and work on service improvement with the practice. Additionally, you will provide expert advice on medications to other clinicians and patients, deliver clinical leadership on medicines optimisation and quality improvement, and ensure appropriate and efficient use of medication. Furthermore, you will educate patients on medication and disease prevention, and create and follow clinical pharmacy programme according to practice policy.
