Clinical Funding Officer / Nurse – Mildura

Southern Cross Care Queensland invites applications for a part‐time Clinical Funding Officer at Oasis Aged Care Home in Mildura. The role is part‐time, guaranteeing a minimum of 30 hours per fortnight. If you are a registered nurse, there may be opportunities to pick up extra nursing shifts.

About the role

In this role you will provide additional Site‐Specific Clinical Funding support to the home for the Australian National Aged Care Classifications (AN‐ACC) with the support of the SCCQ Clinical Funding team. Responsibilities include:

* Liaising with carers and other clinical staff to ensure accurate documentation of resident care needs and time spent, supporting appropriate staffing reflective of residents' care needs.
* Assisting and supporting staff to ensure they have the resources and knowledge of the systems and raising any concerns with the Clinical Manager.
* Upskilling Residential Aged Care staff in the funding process by providing training sessions to relevant roles (RN/EN/PCW/Lifestyle Assistants and Pastoral Carers) as scheduled.
* Providing ongoing education through toolbox sessions at shift change‐over to increase awareness using developed toolboxes and incorporating them into the induction program of new employees.
* Actively participating in monitoring resident conditions, clinical meetings, feedback from clinical teams, handovers and head‐of‐department meetings to gather information around potential reclassification opportunities (including attendance at HODS meetings).
* Assisting with monitoring and tracking residents' data, including all movements, Medicare events and AN‐ACC lodgments; active involvement in the admission process, including pre‐screening, AN‐ACC funding predictions, initiation of assessment and care plans and communication with the clinical team.
* Reviewing assessments and care plans to ensure congruency and that they reflect residents' needs for AN‐ACC Classification.
* Delivering high quality, effective documentation, reporting and tracking; monitoring occupancy, entries and departures.
* Completing pre‐screening/new admission/reclassification forms with all required data to support forecasted classification and comments.
* Completing allocated KPIs for resident screening and reclassification requests each week to support site KPIs for PRPD.
* Actively communicating with internal stakeholders, including attendance at HODS and AN‐ACC meetings and sound communication with the Clinical Funding Manager.
* Performing analysis, data collection and reporting specific to Assessment Management Organisation (AMO) visits, ensuring AN‐ACC assessors have designated areas to complete a review, required logins and all relevant information, and preparing staff for interviews as required.

What you will bring to the role
* Experience in a care or clinical role within a residential aged care facility.
* Knowledge and understanding of the AN‐ACC tools, AN‐AC funding guide and processes in a residential aged care facility.
* Proven ability to quickly acquire knowledge of diverse software systems with a working knowledge of the Microsoft suite.
* Ability to work as part of a team and with minimal supervision.
* Attention to detail and strong customer service skills and ethic.
* Strong communication skills, both written and oral.
Employment details

Type: Part‐time, minimum 30 hours per fortnight. Salary: Competitive (details disclosed upon request).
