Job Overview
We are seeking a dedicated individual to fill the role of Sewb Case Worker — Community Healing.
This position involves supporting the health and resilience of Aboriginal and Torres Strait Islander peoples through personalized case management and advocacy.
You will work closely with individuals and families, engaging in culturally appropriate initiatives while collaborating with a multidisciplinary team to enhance wellbeing across the region.
About This Role
* Supporting Clients
Provide support and guidance to clients to improve their social and emotional wellbeing.
* Collaborative Teamwork
Collaborate with other healthcare professionals to develop and implement care plans.
* Community Outreach
Engage in community outreach and education activities to promote health and wellbeing.
Requirements
To be successful in this role you will need to have excellent communication and interpersonal skills, with the ability to work effectively with diverse communities. A strong understanding of cultural safety principles and practices is also essential.
The ideal candidate will have experience working in a similar role, preferably within a community health setting. They will also possess a sound knowledge of case management principles and practices, as well as effective communication and interpersonal skills.
