We encourage people of Aboriginal and Torres Strait Islander background and people with disability to apply.About the RoleAs a Care Coordinator you will oversee and enhance the delivery of our Aged Care services from North Brisbane to Caboolture.
Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs.
You will facilitate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems.
You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering growth among our aged care clients.Our ideal candidate will be located in or close to Caboolture to be local to the client base they will be supporting.
This is a hybrid working opportunity where you will be able to work some days each week from your own home whilst travelling to visit clients in their homes around North Brisbane up to Caboolture.Key ResponsibilitiesUndertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholdersEnsure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visitsCollaborate effectively with internal stakeholders to ensure clients' needs are met including our Rostering team and Practice LeadsDevelop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communitiesRepresent Life Without Barriers at external forums and meetings, promoting the organisation within the communitySkills & ExperienceDemonstrated Aged Care or other clinical case management experience with strong administration skills, and ability to work with individuals, families and/or stakeholdersUnderstanding of Aged Care fundingHigh level competency in utilising a variety of technology including mobile devices and computersAbility to identify and implement risk mitigation strategiesHigh level communication, influencing, liaison and negotiation skillsEstablished problem solving and conflict management skillsAbility to multi-task and work autonomously in a fast paced environmentA current unrestricted Australian Driver Licence, reliable vehicle and a willingness to travelThorough induction programCareer pathways across a national organisationAttractive remuneration package with a monthly rostered day off (Think 12 extra long weekends a year!
