Disability Support Role
This position involves working directly with individuals living with a disability and/or mental health condition to help them increase their independence, maintain their wellness and ultimately improve the quality of their life.
Key Responsibilities
* Provide case management services to individuals living with a disability and/or mental illness in accordance with service agreements and plans.
* Utilize evidence-based practices in the development and implementation of individual support plans.
* Work one-on-one with clients, providing medication assistance and basic personal care as required.
* Complete, maintain and store documentation in a confidential manner.
Required Skills and Qualifications
* Good availability across multiple days, evenings and overnight shifts per week.

This role requires maintenance of a positive Working with Children (Blue Card), NDIS Worker Screening Clearance, and Current Australian Driver's License.
