Ozcare has a full-time Care Manager position available at our Townsville Community Care branch. We are looking for a compassionate individual who can support our older generation in living independently.
Your Role
Responsibilities
You'll be responsible for planning, managing, and leading client care and support across various programs. You will coordinate Ozcare's multidisciplinary teams and other services to ensure effective care. As a Care Manager, you will:

Manage and coordinate care services to maintain high standards of care
Develop and implement care plans tailored to individual needs
Liaise with clients, families, and healthcare professionals for comprehensive care
Build strong relationships with clients, stakeholders, and team members, assisting clients in meeting their goals
Ensure compliance with relevant legislation and organizational policies
Monitor and evaluate care services for continuous improvement
Oversee the budget of Home Care Packages and ensure clients understand their funding and fees

Requirements for Success

Experience in case management and client care in Community Aged Care
Strong communication skills (verbal, written, interpersonal)
Proof of immunity or willingness to vaccinate against MMR, Varicella, and Pertussis
Current Police Screening Certificate
Current First Aid and CPR Certificates
NDIS Worker Screening Check and Blue Card
Queensland driver's license
Relevant tertiary qualifications or experience in health-related fields
