Nurse Practitioner Regional Palliative Care Consultancy

The Regional Palliative Care Consultancy Nurse Practitioner will be a valued member of the East HUME Regional Palliative Care Consultancy service. The Nurse Practitioner will join a team comprising Palliative Care Physician & Registrar, Nurse Practitioner Candidate in Palliative Care & Grief & Bereavement Worker. The team travels across the east Hume region to provide primary health care providers in community and hospital settings, along with their patients, with specialist palliative care clinical consultation to enhance quality of life, alleviate suffering, and support the holistic needs of patients and their families. The team provides strong clinical leadership for palliative care across the region, delivering education and training, clinical mentoring, and driving improvement in clinical safety and quality. We are actively seeking candidates who meet the following requirements to apply for this exceptional opportunity:
* Current registration and endorsement with AHPRA as a Nurse Practitioner of Palliative Care
* Demonstrated ability to work autonomously and collaboratively in an advanced and extended clinical role
* Effective communication skills with a range of healthcare professionals
* Ability to engage with and promote reflective practice among the nursing workforce
* Capability to apply and promote evidence-informed practice at the point of care
