About the Role
This role is pivotal in ensuring safe, evidence-based care and mentoring teams to deliver exceptional outcomes. The Healthcare Leader will be responsible for:
* Identifying and managing high-risk health concerns across multiple sites
* Developing and implementing health promotion activities tailored to meet client needs
* Collaborating with customers, families, and teams to develop and monitor individualized healthcare plans
* Providing NDIS-funded nursing supports within their scope and training
* Conducting site rotations and spot checks of documentation to ensure compliance and accuracy
* Liaising with hospitals, GPs, and allied health professionals to facilitate coordinated care
* Supporting new clients through clinical assessments and transitions into care
* Maintaining accurate and up-to-date records for quality improvement purposes
Requirements
To succeed in this role, you will require:
* Current RN registration with AHPRA
* Experience in disability or community care
* Strong clinical skills, particularly in high-intensity support environments
* Leadership capability and adaptability in a dynamic environment
* Ability to travel between locations as required
* Current NDIS Worker Screening Check and Working with Children Check
* Current NSW Driver's Licence
* Current Police Check (issued within the last 3 months)
