Job Title: Clinical Care Coordinator
We are seeking an experienced and compassionate Clinical Care Coordinator to join our multidisciplinary team at Helping Hand. The ideal candidate will be responsible for delivering high-quality care services to residents in accordance with their individual needs and care plans.
* Assist the Registered Nurse in planning, implementing, and evaluating nursing and personal care strategies.
* Contribute to the formulation of client care plans in collaboration with other members of the healthcare team.
* Provide care in accordance with relevant legislation, contemporary care standards, and organization policies and procedures.
The successful candidate will possess:
* A diploma in Enrolled Nursing or related field.
* Current AHPRA registration.
* Excellent written communication skills.
* Proficiency in using health information technology systems.
* Ability to communicate effectively with people from diverse backgrounds.
Key Requirements:
* Previous experience in a clinical setting is highly desirable.
* Knowledge of contemporary care practices and standards.
