Make a meaningful difference supporting older people to regain independence and achieve their goals following hospital discharge within our Transitional Aged Care Program.
About the Opportunity
The Occupational Therapist Level 1 will deliver high quality, client centred services within the Transitional Aged Care Program across community and residential settings. This time-limited, goal-oriented program supports older people at the end of a hospital episode to optimise function and plan for longer term care needs. This role will work closely with other teams within the Directorate including CAH (Community Allied Health). In this role you will:
* Provide comprehensive assessment and evidence-based intervention to optimise functional capacity and support the restorative process.
* Develop and implement goal-directed treatment plans in collaboration with clients, carers and the multidisciplinary team.
* Support clients to access and finalise appropriate longer term care arrangements where required.
* Apply professional judgement in clinical decision making and seek guidance from senior clinicians as appropriate.
* Contribute to continuous improvement, professional supervision and adherence to professional, legal and ethical standards.

About You
We are looking for someone who has:
* Degree or Diploma in Occupational Therapy and current registration with AHPRA.
* Demonstrated experience in aged care, rehabilitation, chronic disease or dementia care.
* Proven ability to conduct standardised and non-standardised assessments and deliver targeted interventions.
* Strong communication skills with the ability to engage clients with cognitive or functional challenges and their carers.
* Ability to prioritise workload, work collaboratively within a multidisciplinary team and commit to ongoing professional development.
