**Job Overview:**
As a healthcare professional, you will be responsible for delivering high-quality, person-centred care to residents. This includes creating care plans with residents and care teams, providing clinical care, managing medications, responding to health changes, liaising with families and relatives, working with other teams, and maintaining records.
**Key Responsibilities:**
* Complete assessments of residents' needs and create comprehensive care plans in collaboration with multidisciplinary teams
* Provide direct patient care, including administering medications, performing procedures, and responding to medical emergencies
* Communicate effectively with residents, families, and healthcare professionals to ensure seamless care delivery
* Work collaboratively with colleagues to achieve positive outcomes for residents
* Document accurately and maintain up-to-date records of residents' care and services provided
**Requirements:**
* Current registration as a Registered Nurse and a valid CPR Certificate
* Experience in aged care or dementia care, preferably with strong clinical assessment skills
* Excellent communication, interpersonal, and organisational skills to build strong relationships with residents, families, and staff
* High level of professionalism, confidentiality, and empathy to handle sensitive matters with care and discretion
* Strong problem-solving, time management, and decision-making skills to prioritise tasks and respond to emergency situations
* Computer proficiency, including experience with electronic healthcare records and documentation systems
