Older Adults: Support HCP consumers with falls prevention, dementia care, and aging-in-place solutions
NDIS Participants: Empower clients with disabilities to achieve their goals
Children & Youth: Address developmental delays and school-based challenges
Responsibilities
- Developing personalized intervention plans
- Prescribing assistive tech and home modifications
- Navigating NDIS, HCP, and CDM funding systems
- Building relationships with clients, families, and care teams

Qualifications
- AHPRA registration
- Passion for client-centered, community-based care
- Excellent communication and problem-solving skills
