About This Opportunity
Establish relationships with clients that encourage self-determination and participation. Provide case management support to clients, make ongoing assessments of client issues, needs, and circumstances from initial intake to exit, and assist clients in accessing support services and establishing community supports.
This is a maximum term part-time position based in Hobart, Tasmania. The role encompasses the delivery of assessment and crisis response. Reporting to the Team Leader/Manager, you will be responsible for identifying risks to clients, staff, and the organisation and bringing them to the attention of the Team Leader as soon as possible.
Key Responsibilities:

* Provide case management support to clients.
* Make ongoing assessments of client issues, needs, and circumstances from initial intake to exit.
* Assist clients in accessing support services and establishing community supports.
* Identify risks to clients, staff, and the organisation and bring them to the attention of the Team Leader as soon as possible.
Requirements:
* A degree qualification in social work, welfare, community development, or related field OR demonstrated high level of sector knowledge and skills attained through previous appointments, service, and study/development.
* A NSW Employee Working with Children Check.
* A current First Aid Certificate.
* A valid NSW Driver Licence.
* Relevant experience in a social service environment.
