The Aged and Disability Clinical Lead is a leadership role within Specialised Support Services team, responsible for comprehensive, holistic clinical assessments to determine client care needs, ensuring a person-centred approach. Providing clinical governance and advice to A&DS, ensuring care is evidence-based, culturally appropriate, and aligned with each client's goals, preferences, and rights. Along with Identifying, assessing, and mitigating clinical risks, implementing best-practice risk management strategies to enhance client safety and service quality.
What you'll be doing:
* Provide advice and support to clients, empowering them to navigate the service system and make informed decisions aligned with their needs and preferences.
* Review and ensure compliance with clinical policies, procedures, and health documentation, including discharge summaries, medical reports, and clinical care feedback for A&DS.
* Support the Care Partner team through case conferencing, clinical care planning, and involvement in client care, including ad-hoc nursing assistance when required.
* Support clinical practices for higher level package clients, in collaboration with Care Partners, ensuring care plans are tailored and effective.
* Work collaboratively with clients to understand their individual needs, goals, and aspirations, ensuring they are actively involved in decision-making about their care.
* Advocate within the organisation and externally to uphold clients' rights, ensuring that services and systems prioritise client choice, dignity, and autonomy.
* Foster a collaborative and inclusive team environment, encouraging open communication, knowledge-sharing, and mutual support among colleagues.
* Actively participate in interdisciplinary teamwork, ensuring seamless service coordination and effective client care.
* In collaboration with the Coordinator Specialised Support Services, communicate in a transparent and constructive manner about challenges affecting clients, the team, or service delivery, while working proactively to find solutions.
* Contribute to the implementation and continuous improvement of the Quality Improvement Program, ensuring best practices in service delivery within Aged & Disability Services.
* Support and uphold Occupational Health & Safety (OHS) standards, actively promoting a safe working environment for staff and clients.
* Play an integral role in the development, implementation, evaluation, and review of Aged & Disability Services policies and procedures, while also supporting service quality to ensure alignment with industry standards and evolving regulations.
* Actively pursue continuous professional development to maintain and enhance clinical expertise and knowledge in alignment with industry best practices.
The skills and experience you'll bring to the role:
* Extensive knowledge and experience in Consumer Directed Care for Aged and Disability clients.
* Experience in conducting clinical assessments and developing, monitoring, and reviewing care plans in alignment with client needs and service delivery guidelines.
* Ability to supervise, mentor, and support less experienced staff, ensuring the delivery of high-quality care and compliance with organisational standards.
* An understanding of relevant aged care policies, legislation, and the Aged Care Quality Standards.
* Well-developed written and verbal communication skills, including the ability to prepare reports, correspondence, and documentation to a professional standard.
Required Certifications & Qualifications:
* Tertiary qualifications in Allied Health, Nursing (RN Division 1), or a related field.
