Your role will help people achieve their goals, improve quality of life, and support both physical and mental well being.

About the Role
* Provide personalised care and support to participants in their own homes. Help clients live independently and safely while building meaningful routines.
* Assist with grooming, toileting, cleaning, meals, and medication
* Follow personalised support and health plans
* Provide companionship and emotional support
* Respect client preferences, privacy, and cultural needs
* Report concerns and changes in condition promptly
About You
* Minimum 3 months' experience or Cert III/IV in Disability, Aged Care, or other related fields is highly desirable
* Experience working in in-home care
* Mental health, personal, and wound care highly desirable
* Strong understanding of professional boundaries, restrictive practices, and behaviour support
* Effective written and verbal communication, including clear case note writing
* Able to set priorities, meet deadlines, and work under pressure with minimal supervision
* Strong interpersonal skills to work both autonomously and as part of a team
* NDIS Worker Screening, First Aid, Driver's Licence, and insured vehicle
