The Role
Based on the Central Coast, you will provide advanced assessment, diagnosis, prescribing, treatment and care planning for residents within Residential Aged Care Homes. You will work within a collaborative multidisciplinary model alongside on-site nurses, General Practitioners, telehealth Geriatricians and Physicians, with dedicated administrative and operational support so your focus remains on delivering high-quality clinical care.
What You'll Bring
* AHPRA registration as a Registered Nurse with Nurse Practitioner endorsement
* Experience in aged care, gerontology, palliative care or chronic disease management highly regarded
* Advanced clinical experience in assessment, diagnosis, treatment and health promotion
* Strong clinical reasoning, communication and leadership skills
* Experience in quality improvement and evidence-based practice
* Sound knowledge of aged care legislation and regulatory requirements
* Current driver's licence, reliable vehicle and professional indemnity insurance
* Comfortable using technology and clinical software systems
Qualifications
* Master of Nursing (Nurse Practitioner)
* Current AHPRA registration as a Registered Nurse with Nurse Practitioner endorsement
