North East Family Medicine is a General Practice Clinic in Wangaratta looking to employ a Registered or Endorsed Enrolled Practice Nurse.

Fixed term contract of 16 hours per week until 5 February ****, with additional hours to cover sick leave and annual leave as required.

Role Summary
Enthusiastic and proactive nurse needed to deliver excellent medical care in a fast-paced, team-focused environment.
You will play a vital role in patient management, clinical support, and practice administration.
Primary Job Tasks and Responsibilities
Immunisations and injections
Assisting with procedures
ECG's
Assistance in care of unwell patients
Venepuncture
Triage
Communicating results to patients
Cold chain compliance
Infection control compliance
Assisting with Accreditation policy and procedure management
Assist with patient care coordination of chronic disease management through the following tasks (as relevant to each individual patient's needs):
Assessment and review of chronic condition status
Provision of patient education and self-management training
Assistance with the preparation and review of GP management plans
Assistance with the preparation of health assessments
Engagement with multidisciplinary case conferences and care planning
Coordination of referrals
Contribution to improvement of practice systems and improvement of patient data collection/management
Qualifications and Experience
Previous experience in General Practice.
Current unrestricted AHPRA registration as an RN/EEN.
Current Working with Children Check.
Compliant with CPD requirements.
Demonstrated risk management and OH&S.
Experience with Best Practice software or similar.
Ability to review and implement continuous improvement programs.
Cold Chain Management Knowledge.
Ability to multitask, prioritise and work effectively in a busy environment delivering care with integrity and accountability.
Team-focused mindset and a positive attitude.
Professional presentation.
Effective clinical communication through the use of information technology, written and verbal means and sound interpersonal skills with the capacity to interact with others in a personable and professional manner.
