About the Role
We are seeking a highly skilled and dedicated professional to join our team as a Youth Case Manager. In this role, you will be working closely with young people who are making the transition from the out-of-home care system to adulthood.
Our team provides individualized supports across a range of life areas including housing, health and wellbeing, education, employment, and community and cultural connections. We are committed to empowering young people to have an active voice in their transition planning and to provide them with the necessary skills and resources to succeed in life.
As a Youth Case Manager, your primary responsibility will be to provide holistic and tailored coaching to young people, in line with the Advantaged Thinking Framework. This will involve developing and supporting individual support plans as per our guidelines, working within the required funding framework, and complying with statutory requirements where necessary.
You will also be responsible for accurately completing and maintaining all necessary records, reports, case notes, and outcomes in accordance with our funding contracts and best practice guidelines. Additionally, you will promote our program at forums, networks, and community agencies, and administer brokerage as approved in line with our guidelines.
