Working with a significant degree of autonomy, you will deliver assessment, intervention, and care coordination for older clients with complex health needs, focusing on hospital avoidance, supported discharge, and hospital substitution. You will work collaboratively within a multidisciplinary team, providing culturally appropriate services in a variety of settings—including community, residential, and home environments. You will play a pivotal role in delivering restorative and resilience-building interventions, supporting clients who experience gaps in traditional health responses. With an emphasis on continuous professional development, you will consolidate your skills from novice to proficient practitioner and take accountability for your clinical standards and delegated activities. About You To excel as a Physiotherapist within CALHN Integrated Care Geriatric Services, you will demonstrate the following key capabilities: Qualified and Registered: Hold a degree or qualification enabling registration as a Physiotherapist with the Australian Physiotherapy Board (AHPRA). Clinical Competence: Demonstrated experience in delivering physiotherapy care in acute, tertiary, and/or community settings, with proven ability to manage clients with complex musculoskeletal and health conditions. Professional Skills: Strong communication (verbal and written), teamwork, and client-centred approaches; proven ability to prioritise, innovate, and apply sound clinical judgement with minimal supervision. Knowledge and Initiative: Well-developed knowledge of evidence-based practice, work health and safety, and a commitment to ongoing professional development, reflective practice, and service improvement.
