Independent Living Support Specialist

Duties and Responsibilities:

* Create personalized care plans, conduct home safety checklists, develop customized budgets, and provide ongoing case management to participants to ensure their needs are met.
* Provide financial and case management services to customers and representatives as required, focusing on delivering high-quality support.
* Build strong relationships with internal and external stakeholders, seeking creative solutions to achieve the best outcomes for clients.
