About the Role - Our clinical team delivers the Advanced Preventative Care (APC) Program across Tasmania. This program empowers our clients living with chronic disease to change their health outcomes. As we continue to grow, we seekan experiencedPermanent Part Time, Registered Nurse Care Manager (0.7 - 0.8 FTE) negotiable to join this team.
Key Responsibilities
* Deliver high-quality nursing care to our clients in the community including assessments, care planning, and ongoing management of preventative care
* Provide clinical expertise and support to clients with chronic conditions
* Assess and identify deterioration and changes in clients' conditions, communicating results of assessment to clients' medical team as appropriate
* Work collaboratively with healthcare professionals, families, and carers to develop and implement individualised care plans
* Provide education, advice, support and goal setting assistance for clients and their families to manage health conditions and promote wellness
* Ensure accurate and timely documentation of care and clinical activities
Essential Criteria
* Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
* More than 2 years industry experience in acute care, aged care, community nursing or similar healthcare setting is desirable
* Demonstrated knowledge and experience in client assessment, care planning and experience in chronic disease management
* Strong clinical and critical thinking skills with a focus on delivering person-centred care
* Ability to work independently, manage a caseload, and use initiative
* Demonstrated ability in maintaining high clinical standards aligned with compliance, governance and the National Safety & Quality Health Service Standards (NSQHSS)
* Excellent communication skills, both written and verbal, with the ability to build rapport with patients, families, and colleagues
* MS Office proficiency
* Demonstrated commitment to ongoing professional development.
* Current driver's license
* Ability to travel to Northern Coast of Tasmania and surrounding areas (e.g Burnie, Penguin, Ulverstone, Devonport)
* A satisfactory National Police Check
* Work with Vulnerable People Card (WWVP)
