About the Role
We are seeking a dedicated Case Manager to join our Specialist Homelessness Services team. In this role, you will provide trauma-informed, person-centred support to individuals and families experiencing or at risk of homelessness. You'll work collaboratively with clients to develop tailored case plans, connect them with essential services, and empower them to achieve sustainable housing outcomes.
Key Responsibilities
* Deliver high-quality case management services to clients with complex needs.
* Conduct comprehensive assessments and develop individualised support plans.
* Advocate for clients and coordinate referrals to housing, health, legal, and social support services.
* Maintain accurate and timely documentation in line with service standards.
* Collaborate with internal teams and external stakeholders to ensure holistic support.
* Contribute to service development and continuous improvement initiatives.

This is a full time fixed term contract Maternity Leave cover to 4/5/2026.

Criteria:
• Tertiary qualification in a social science or related field and/or specialised skills sufficient to perform at this level.
• Current Driver's Licence.
• Current Working with Children Check (NSW).
• Satisfactory Criminal Record Check
• Casework experience in an associated field.
• Understanding and experience of working with Aboriginal people and their communities.
• Understanding of, and demonstrated ability to work with Human Service Agencies.
• Demonstrated experience and/or understanding of Trauma Informed Care.
