Star Disability and Medical Services is a registered NDIS Service Provider established in
2019 and servicing The Northern Territory-Darwin, Perth and Western Australia,
Australian Capital Territory from a couple with experience in Disability and the aged care sectors. Star Disability and Medical Services now operates accommodation and community supports across three states.
Position Context
The purpose of this position is to provide the highest level of support to individuals utilising our services. This will be achieved through the implementation of services and programs that are flexible and individualised in response to each person being supported.

2.2 Service Delivery
* Participate in a range of appropriate programs and activities (recreational, pre-vocational, education and social) aimed at increasing the individual's skills and access to and participation in the community
* Provide the highest level of support for all individuals supported and ensure each person supported is treated with respect that enhances their personal worth, dignity and rights, at all times
* Assist in achieving individual's, team and service goals and objectives.
* Seek ways of improving the standards of service by working closely with colleagues, relevant managers and contributing to meetings
* Maintain a high level of service for all individual's and ensure the health, wellbeing and safety of all people supported and staff is always maintained

* Support the identified needs of individuals in relation to their life circumstances
* Ensure that where required attendance to personal care needs of clients is undertaken in a professional, empathetic, and timely manner
* Maintain handwritten and electronic records including but not limited to; file notes and incident reports, Person Centred Plans (PCP); OH&S, and other relevant documentation with confidentiality and accuracy in relation to stated objectives, needs assessment and life circumstances
* Contribute to the development of team action plans
* Ensure you provide a safe working environment
* Carry out the role of Key Worker as per Accord's guidelines
* Act as a professional and practical role model
* Participate in regular supervision, attend, and contribute to staff meetings
* Always ensure that you are appropriately attired which will be informed by the organization's Dress Code
* Administer medication as required and according to policy and procedure
* Uphold the Code of Conduct and Confidentiality Statement in all your decisions, behaviours and actions
* Comply and adhere with policies procedures and workplace instructions ensuring best practice in service delivery is achieved
* Participate in the review and implementation of PCP's and behaviour support plans
* Participate in the annual Performance Review and Development process
* All other reasonable duties as directed by Manager/Supervisor.

4. Communication
* Communicate effectively with all customers and patients using the appropriate channels, utilising appropriate formal and informal channels of communication.

* Participate in, contribute to and implement quality improvement and risk management into all aspects of service

* Adhere to all applicable policies and guidelines.
* Take reasonable care of themselves and others.
* Not to interfere, bypass or misuse any system or equipment provided for health, safety, and welfare purposes.
* Take all actions to avoid, eliminate or minimize hazards.
* Seek information on any work they undertake and be aware of the risks and hazards associated with their work.
* Report all incidents / hazards / injuries.
* Assist in completion of incidents / hazards / injuries reports.

* Participate in the documentation of Risk Assessments and Standard Operating Procedures for activities that could pose an OHS risk.
* Raise OHS issues with appropriate caregivers and take part in OHS consultative arrangements.
* Use Personal Protective Equipment as required and directed.
* Attend all mandatory and recommended OHS training as scheduled by Group Services.
* Promote a positive safety culture within their areas by demonstrating a positive commitment to OHS.
