This is an excellent opportunity for a motivated and organised coordinator to be an integral part of our CHSP client's journey, including intake and referral, onboarding, and ongoing assessment needs, all whilst ensuring a positive experience for our clients that supports their independence.Key ResponsibilitiesSupport all aspects of client coordination and ensure clients' needs are being met with an exceptional and effective serviceReview care plans, update service activities and liaise with relevant internal stakeholdersProvide a client-centred experience and ongoing care support serviceLiaise and build rapport with clients, their families, stakeholders, and staff to ensure an exceptional and effective service is deliveredEnsure timely and accurate processing of client records and databasesProvide an excellent standard of customer service, ensuring sensitive client interactions are handled with respect, care, and confidentialityAbout youYou have passion with purpose and are driven by excellence.
